PARENT CONSENT *
PARENT RESPONSIBILITY – I take full responsibility for the safety of my child immediately following the conclusion of the program. If my child is not picked up immediately following the conclusion of the program, my emergency contact will be called. I agree not to hold any of the ArtHouse venue providers responsible for any harm or injury to my child that may result during the time spent at ArtHouse Programs. I have read the Front of the Line Policy created to offer priority registration opportunities to children whose families cannot access fee-related Arts Programs. *

PARENTS - PHOTO/AUDIO RELEASE FORM FAMILY - PHOTO/AUDIO RELEASE FORM - from time to time ArtHouse and/or its Community Partners may take and use photographs and/or audio-video recordings of its young participants and family members to help build awareness about the program and to encourage donor support so that we can continue to provide cost-free programs. I consent to the use of my photographic image and those of our family members in printed material or other media used by ArtHouse in connection with its programs, advertising, fundraising or other public relations activities. Such use shall be without compensation to me. This consent shall be in effect until it is revoked at any time in writing. If it is revoked, such revocation will not affect any use of my image (my child’s image) while this consent was still in effect. I have read the above and agree with its contents: *
